The use of a magnetic nanoparticle tracer and a handheld magnetometer provides a radiation-free alternative, which was recently evaluated in two clinical trials. The hydrodynamic particle size of the used magnetic tracer differs substantially from the radioisotope tracer and could therefore benefit from optimization. The aim of this study was to assess the performance of three different-sized magnetic nanoparticle tracers for sentinel lymph node biopsy within an in vivo porcine model.Materials and methods: Sentinel lymph node biopsy was performed within a validated porcine model using three magnetic nanoparticle tracers, approved for use in humans (ferumoxytol, with hydrodynamic diameter dH =32 nm; Sienna+®, dH =59 nm; and ferumoxide, dH =111 nm), and a handheld magnetometer. Magnetometer counts (transcutaneous and ex vivo), iron quantification (vibrating sample magnetometry), and histopathological assessments were performed on all ex vivo nodes.Results: Transcutaneous “hotspots” were present in 12/12 cases within 30 minutes of injection for the 59 nm tracer, compared to 7/12 for the 32 nm tracer and 8/12 for the 111 nm tracer, at the same time point. Ex vivo magnetometer counts were significantly greater for the 59 nm tracer than for the other tracers. Significantly more nodes per basin were excised for the 32 nm tracer compared to other tracers, indicating poor retention of the 32 nm tracer. Using the 59 nm tracer resulted in a significantly higher iron accumulation compared to the 32 nm tracer.Conclusion: The 59 nm tracer demonstrated rapid lymphatic uptake, retention in the first nodes reached, and accumulation in high concentration, making it the most suitable tracer for intraoperative sentinel lymph node localization. It is, however, the most common cancer in young men in Western populations. The incidence of testicular cancer is increasing globally, although a decline in mortality rates has been reported in Western countries. It is important to identify whether the variations in trends observed between populations are linked to genetic or environmental factors.Methods: Age-standardized incidence rates and age-standardized mortality rates for testicular cancer were obtained for men of all ages in ten countries from the Americas, Asia, Europe, and Oceania using the Cancer Incidence in Five Continents (CI5plus) and World Health Organization (WHO) mortality databases. The annual percent change was calculated using Joinpoint regression to assess temporal changes between geographical regions.Results: Testicular cancer age-standardized incidence rates are highest in New Zealand (7.8), UK (6.3), Australia (6.1), Sweden (5.6), USA (5.2), Poland (4.9), and Spain (3.8) per 100,000 men. India, People’s Republic of China, and Colombia had the lowest incidence (0.5, 1.3, and 2.2, respectively) per 100,000 men. The annual percent changes for overall testicular cancer incidence significantly increased in the European countries Sweden 2.4%, (2.2; 2.6); UK 2.9%, (2.2; 3.6); and Spain 5.0%, (1.7; 8.4), Australia 3.0%, (2.2; 3.7), and People’s Republic of China 3.5%, (1.9; 5.1). India had the lowest overall testicular cancer incidence -1.7%, (-2.5; -0.8). Annual percent changes for overall testicular cancer mortality rates were decreasing in all study populations, with the greatest decline observed in Sweden -4.2%, (-4.8; -3.6) and People’s Republic of China -4.9%, (-6.5; -3.3).Conclusion: Testicular cancer is increasing in incidence in many countries; however, mortality rates remain low and most men are cured. Abstract

New statistical methods were employed to improve the ability to distinguish benign from malignant breast tissue ex vivo in a recent study. The ultimately aim was to improve the intraoperative assessment of positive tumour margins in breast-conserving surgery (BCS), potentially reducing patient re-operation rates. A multivariate Bayesian classifier was applied to the waveform samples produced by a Terahertz Pulsed Imaging (TPI) handheld probe system in order to discriminate tumour from benign breast tissue, obtaining a sensitivity of 96% and specificity of 95%. We compare these results to traditional and to state-of-the-art methods for determining resection margins. Given the general nature of the classifier, it is expected that this method can be applied to other tumour types where resection margins are also critical.

Abstract

In heterogeneous cohorts and those where censoring by non-primary risks is informative many conventional survival analysis methods are not applicable; the proportional hazards assumption is usually violated at population level and the observed crude hazard rates are no longer estimators of what they would have been in the absence of other risks. In this paper, we develop a fully Bayesian survival analysis to determine the probabilistically optimal description of a heterogeneous cohort and we propose a novel means of recovering hazard rates and survival functions `decontaminated' of the effects of any competing risks. Most competing risks studies implicitly assume that risk correlations are induced by cohort or disease heterogeneity that is not captured by covariates. We additionally assume that proportional hazards hold at the level of individuals, for all risks, leading to a generic statistical description that allows us to decontaminate the effects of informative censoring, and from which Cox regression, frailty and random effects models, and latent class models can all be recovered as special cases. Synthetic data confirm that our approach can map a cohort's substructure, and remove heterogeneity-induced false protectivity and false aetiology effects. Application to survival data from the ULSAM cohort leads to plausible alternative explanations for previous counter-intuitive inferences to prostate cancer. Abstract

Background The role of adjuvant therapy in patients with oesophagogastric adenocarcinoma treated by neoadjuvant chemotherapy is contentious. In UK practice, surgical resection margin status is often used to classify patients for receiving adjuvant treatment. The aim of this study was to assess the survival benefit of adjuvant therapy in patients with positive (R1) resection margins. Methods Two prospectively collected UK institutional databases were combined to identify eligible patients. Adjusted Cox regression analyses were used to compare overall and recurrence-free survival according to adjuvant treatment. Recurrence patterns were assessed as a secondary outcome. Propensity score-matched analysis was also performed. Results Of 616 patients included in the combined database, 242 patients who had an R1 resection were included in the study. Of these, 112 patients (46*3 per cent) received adjuvant chemoradiotherapy, 46 (19*0 per cent) were treated with adjuvant chemotherapy and 84 (34*7 per cent) had no adjuvant treatment. In adjusted analysis, adjuvant chemoradiotherapy improved recurrence-free survival (hazard ratio (HR) 0*59, 95 per cent c.i. 0*38 to 0*94; P= 0*026), with a benefit in terms of both local (HR 0*48, 0*24 to 0*99; P= 0*047) and systemic (HR 0*56, 0*33 to 0*94; P= 0*027) recurrence. In analyses stratified by tumour response to neoadjuvant chemotherapy, non-responders (Mandard tumour regression grade 4-5) treated with adjuvant chemoradiotherapy had an overall survival benefit (HR 0*61, 0*38 to 0*97; P= 0*037). In propensity score-matched analysis, an overall survival benefit (HR 0*62, 0*39 to 0*98; P= 0*042) and recurrence-free survival benefit (HR 0*51, 0*30 to 0*87; P= 0*004) were observed for adjuvant chemoradiotherapy versus no adjuvant treatment. Conclusion Adjuvant therapy may improve overall survival and recurrence-free survival after margin-positive resection. This pattern seems most pronounced with adjuvant chemoradiotherapy in non-responders to neoadjuvant chemotherapy.
